Request for School Compliance Verification Form (Learner's Permit)

To request your School Compliance Verification (KRS 159.051), please have either the student or guardian complete the form below. This verification, necessary for students to test for their learner's permit, must be signed by the school and is contingent upon the student's school attendance (fewer than 9 unexcused absences) and passing 5 of 7 courses or their equivalent in the preceding semester. Please note that the validated School Compliance Verification form expires 60 days after completion. Allow three school days for the Lexington Catholic registrar to process this form.
